To help make your first visit to our practice as smooth as possible, please read the information on this web page. We describe our first visit procedures and you can save some time by completing the new patient forms in advance of your appointment.
Once you have secured an appointment, we ask that you register within our Patient Portal, located on the top right of your screen. This will expedite your check-in process. If you fill out the registration information within the portal completely and correctly, you do not need to complete the paper version of the “Patient Registration Form” which will be mailed to you in advance of your appointment.
What to expect
Please bring your driver’s license/identification card and insurance card with you to every visit. Be prepared to make your insurance copay or office visit payment prior to your visit. If you have had a recent CAT scan at a facility other than Memorial Hospital, please bring a disc of the images to your appointment, if possible. If you have had an audiogram or allergy testing at another facility in the past, please bring a copy of the results. It is extremely important to bring a list of your current medications with you to every visit.
If you arrive more than 10 minutes late to your scheduled appointment, you will be asked to reschedule.
